Social Work Program Coordinator (Community Nursing Home Program)

Duties: This position is assigned to the Social Work Service and the incumbent will serve as The Community Nursing Home (CNH) Program Coordinator.

The incumbent is administratively responsible for the Community Nursing Home Program (CNH), Community Adult Day Care Program, Homemaker Home Health Aide (HHA) and Non-Institutional Respite (NI-R). The CNH Coordinator manages the daily operation of the program and direct patient care services in the program area. S/he develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. S/he is responsible for monitoring the program's budget and various control points assigned to the program within the assigned areas.

The incumbent manages and controls the use of data related to these programs regarding day to day functioning and program evaluation. In conjunction with the Service Chief, or designee, the incumbent plans, organizes and directs administrative, operational patient concerns and oversight of program resources in the community, liaison with community resources, staff education regarding CADHC services and referral processes, and the development, evaluation and communication of standards, policies and procedures. This is accomplished in collaboration with other members of the CADHC team or other interdisciplinary treatment teams as appropriate (i.e. primary care, inpatient services, specialty clinics, CLC, mental health, etc.). The incumbent collaborates with the Chief, Social Work Service in policy development and with regard to funding expenditures. The incumbent tracks resource utilization and resource allocation decisions. The CNH Coordinator has specialized knowledge of practice typically related to a particular diagnosis or population.

Qualifications & Evaluations BASIC REQUIREMENTS. The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000.
